Background

The Population Health Management Task Force is made up of 13 National Training and Technical Assistance Partners (NTTAPs). Funded by HRSA’s Bureau of Primary Health Care (BPHC), the NTTAPs provide subject matter expertise and specialized training and technical assistance (T/TA) focused on a specific population type or health center operation.

The Population Health Management Task force is comprised of Association of Asian Pacific Community Health Organizations (AAPCHO), Corporation for Supportive Housing (CSH), Farmworker Justice (FJ), Health Outreach Partners (HOP), MHP Salud, Migrant Clinicians Network (MCN), National Center for Equitable Care for Elders (NCECE), National Center for Farmworker Health (NCFH), National Center for Health in Public Housing (NCHPH), National Health Care for the Homeless Council (NHCHC), National LGBT Health Education Center (Fenway), National Network for Oral Health Access (NNOHA), and National Nurse-Led Care Consortium (NNCC). Association for Asian Pacific Community Health Organizations (AAPCHO) promotes advocacy, collaboration, and leadership to improve the health of Asian Americans, Native Hawaiians, and Pacific Islanders (AA & NH/PI). AAPCHO can provide technical assistance and expert consultation to support and build the capacity of health centers and organizations serving AAs and NH/PIs.

Corporation for Supportive Housing (CSH) works at the intersection of health and housing. CSH offers training and technical assistance to address the health and social needs of patients experiencing homelessness or living in public housing.

Farmworker Justice (FJ) works to improve health care and address the health and occupational safety needs of migrant and seasonal farmworkers and their families.

Health Outreach Partners (HOP) focuses on outreach and enabling services for migrant and seasonal farmworkers and their families. Areas of focus include transportation, program planning and development, needs assessments and evaluation, and community collaboration.

MHP Salud helps health centers to develop and sustain Promotoras de Salud (Community Health Workers) programs through health education and community outreach development.

Migrant Clinicians Network (MCN) addresses all aspects of clinical care affecting migrant and seasonal agricultural workers and their families. TA is directed to patients, providers, and clinic systems through consultation, patient tracking, bridge case management, and clinical education.

National Center for Farmworker Health (NCFH) specializes in health center governance, administration, and patient education that enhance health centers’ delivery of services for migrant and seasonal agricultural workers and their families.

National Center for Equitable Care for Elders (NCECE) offers innovative culturally and age-appropriate models of care as well as interprofessional training and education to health care professionals in community health centers that serve older adults.

National Center for Health and Public Housing (NCHPH) works to improve primary care professionals’ ability to recognize and address the specialized health and social needs of public housing residents. T/TA increases patient health literacy and health care access. T/TA also improves culturally appropriate service delivery and health equity.

National Health Care for the Homeless Council (NHCHC) helps health centers respond to the needs of people experiencing homelessness or who are unstably housed. T/TA includes chronic and acute medical management and social and health care coordination.

National Network for Oral Health Access (NNOAH) works to develop, expand, and integrate oral health services into primary care settings while adopting effective practices to monitor, report, and improve oral health.

National Nurse-Led Care Consortium (NNCC) helps public housing-based health centers to improve clinical practice to meet the needs of public housing residents.
