Background

The Special and Vulnerable Populations Diabetes Task Force was an initiative comprised of 14 National Training and Technical Assistance Partner (NTTAP) organizations formed in 2017. In alignment with HRSA’s Diabetes Control objective, the collective goal of the Taskforce was to provide health centers with training and technical assistance (T/TA) to decrease the number of diabetic patients with an A1C greater than 9% (HbA1c > 9%). Since 2017, the Task Force has coordinated webinars on topics related to diabetes, social determinants, and co-morbidities. It also shared resources and strategies among the NTTAPs and HRSA to coordinate T/TA to increase knowledge of effective strategies that address diabetes among people experiencing homelessness, residents of public housing, migratory and seasonal agricultural workers, school-aged children, older adults, Asian Americans, Native Hawaiians and Pacific Islanders, LGBTQIA+ people, and other health center patients.

This project was funded from 2017 - 2023 through the National Training and Technical Assistance Partner Cooperative Agreement. This website was formed as a compendium of resources following the completion of the 2020-2021 project year and contains content from 2020 - 2023. All content is considered archived and has not been updated since the publish date.

The Special and Vulnerable Population Diabetes Task Force collectively coordinated three series of T/TA activities to understand and address key issues related to effective diabetes continuum of care for special and vulnerable populations in Health Center Programs. Each Series consisted of a four-part National Learning Series (NLS) and four multi-session Learning Collaboratives (LC). In 2020, the Task Force developed a joint website (chcdiabetes.org) to serve as a compendium of resources for materials related to the NLS sessions and LC series. At the end of Years 2 and 3, the Task Force produced a compendium of resources for each topic represented in the related NLS sessions and LC series.
